Consignment Note samples

Consignment note shall be exclusively those distributed by the railway company. (can be obtained at the cargo cash-desks of railway stations). The forwarding agent shall be responsible for any entries recorded in the consignment note even if, upon its request, an employee completed the consignment note but the customer checked and signed it and provably made those information available.

Mandatory/Minimum content of the consignment note:

• Name and address of the customer or sender or both (if applicable)
• Name of dispatching and destination station in printed without abbreviation
• Title of goods (NHM code), weight, qty, item code
• Indication of the party performing loading and unloading 
• Packaging characteristics of the consignment 
• Applicable payment method, cost-bearer, prepayment regulation 
• Detailed list of instruments required by authorities that are enclosed to the consignment note if accompanying documents are required for the transportation of goods of for handling purposes at authority checks during transportation.
• Statement of the customer on acceptance of the terms and conditions unless otherwise agreed. 

It shall also include (on demand):

• Item No, quantity and weight of containers and loading tools used 
• Name and classification of hazardous goods as specified in RID regulation
• COD amount 
• Value estimation 
• Statement on the interest in despatch
• Type, ID No and quantity of seals used

These shall be completed in multiple copies. Two copies shall be provided to the dispatching railway company and each transit railway company involved in the freight shall receive one copy.
